The Wisconsin Partnership for Health Payment Reform: More than 100 work group participants are engaged in designing and implementing payment reform pilot projects for in the areas of Acute Care (Total Knee Replacement) and Chronic Care (Diabetes). In this approach, hospitals and physicians take responsibility for the full continuum of care received by their patients. The payment model combines a per-patient global budget with significant performance incentives based on nationally endorsed quality measures over a five year period.
